Basic Multi-Language AJAX / PHP web Contact Form

Configurable / Optional inclusion of Email Recipient Routing, Checkbox Group, Radio Button Group, Multi Part Date, SQL Date with date picker
Additional checkbox, radio buttons, and date fields can be configured using working examples
Temporarily enable the debug mode to suppress processing and display the $_POST array
Approximately 50 English key/value pairs (or messages) included
Multi-language capability requires your translation of approximately 50 English key/value pairs and messages
Includes minimal Javascript examples for catching empty entries and displaying an alert
Secondary validation accomplished with PHP (re-displaying error messages in red above individual form fields)
This application *only* works as intended if the user's browser supports Javascript and cookies

Requires sessions to set and hold the language abbreviation in $_SERVER['lang']
Generally suitable for adaptation to other types of public forms, not just Contact forms
PHP 5 recommended (errors logged only if function file_put_contents() exists)
